Your initial appointment will be 60 minutes. Your follow up appointment will be 30 minutes.
Please plan to arrive 10 minutes prior to your initial appointment to register, review our office policies, and have your child’s growth measured.
Please bring your child’s valid Alberta health care card to your appointment
Paid parking is available in the main parking lot at the front of the building.
Please arrange sibling care for your other children. We would like to focus our attention on the child who requires the appointment.

Many patients are waiting for an appointment. If you must cancel or reschedule for any reason, please call us at least 24 hours prior to your appointment so we can accomodate another patient.
If you missed your appointment, a fee of $300.00 (initial appointment) and $100.00 (follow up appointment) will be required to be paid prior to re-scheduling your appointment.
Alberta Health and Wellness does not cover all services.
Please discuss with our staff at check-in if you request an uninsured service.
Fees are in accordance with guidelines suggested by Alberta Medical Association.
Prescription renewals requested outside of an office visit: $20.00 for 1st prescription plus $5.00 for each additional medication refilled at the same time
Blue Cross authorization forms: $50.00
Disability Tax Credit Certificate: $100.00
Third-party requested letters on patient attended: $225.00-460.00
APS - for insurance company: $130.00-260.00
Note of fitness or illness: $50.00
School requested forms: $70.00-100.00
Driver's medical form: $100.00
Chart copies: $25.00 basic up to 20 pages, then $0.25/p
